Liability Release Waiver and Consent for Training
In consideration for allowing me to participate in a Discovery Flight, flight training or other flying activities with Robert Haynes and Associates, LLC dba Stick and Rudder Aviation (RHA), I hereby agree to the Waiver and Release of Liability. (you will electronically sign this after you submit this form).
I understand that I must have at least a private pilot certificate in order to receive flight training and that all training is dual instruction only. I understand that I do not need to be current or have a current medical certificate to receive training. I agree to fully disclose my reasonable weight to confirm eligibility to meet required weight/balance restrictions. I will present logbook and/or certificates if asked to do so. I further agree to pay the charges invoiced to me by RHA upon receipt (including applicable bank card fees, if appropriate). I understand that RHA typically video/audio records all flight lessons for training and archival purposes, but I will not have access to those video/audio recordings. RHA does not make any video available to the general public without the expressed email consent of the client. I give RHA authorization to publish my photo on their website or facebook page at the completion of training unless I specifically instruct RHA not to do so.
